TLV Medical Says Wrong Substance Used in Teen's Nose Surgery

TLV Medical said Sunday that operating-room staff used tampons soaked in the wrong substance during an 18-year-old patient's nose surgery, calling it an exceptionally rare human error by a nurse. The clinic said staff detected the mistake within seconds, transferred her to Ichilov Hospital and reported the event to Health Ministry authorities. Earlier reports said she suffered severe facial and airway burns, remained sedated and ventilated after emergency surgery, and police opened a negligence investigation. The Health Ministry said it had not been notified, creating a factual dispute with the clinic's statement.
