TLV Medical Says Wrong Substance Used in Teen's Nose Surgery

TLV Medical said Sunday that operating-room staff used tampons soaked in the wrong substance during an 18-year-old patient's nose surgery, calling it an exceptionally rare human error by a nurse. The clinic said staff detected the mistake within seconds, transferred her to Ichilov Hospital and reported the event to Health Ministry authorities.
